Collaborative Health Partners is pleased to work with Walk-In Care’s newest division, Workplace Health. Walk-In-Care, in partnership with Rehab Associates, is expanding the delivery of care for employers’ occupational and employee health needs in the Central Virginia area. We offer a wide array of services to employers:

Collaborative Health Partners of Virginia (CHP) and its primary care partners—CVFP Medical Group, Walk-In-Care, and Physicians Treatment Center—are pleased to announce their success as a participant in one of the nation’s highest-performing accountable care organizations (ACO), Privia Quality Network (PQN). PQN achieved shared savings of $57 million through the Medicare Shared Savings Program (MSSP). Together, Privia Health ACOs provided high-value, cost-efficient care to more than 119,000 Medicare beneficiaries in 2019.

In an effort to continue building on CHP’s commitment to value, CHP is expanding its services for Direct Primary Care, as well as Direct Care, to include a variety of medical services for both employers and general consumers.

Collaborative Health Partners of Virginia (CHP) and its primary care partners, CVFP Medical Group, Walk-In-Care, and Physicians Treatment Center are pleased to announce their success as a participant in one of the nation’s highest-performing accountable care organizations (ACO), Privia Quality Network (PQN). Privia Health’s ACOs across the national PQN delivered quality care and improved patient outcomes, saving Medicare and the American healthcare system more than $70 million through the MSSP.

Liberty University has announced an enhanced primary care benefit to its employees. Through a contract with Collaborative Health Partners, a clinically integrated physician network of primary care providers, Liberty will offer a Direct Primary Care (DPC) program designed to improve employees’ scope of health benefits while reducing their projected healthcare spending. The program aims to increase access to preventive care, sick care, physician-directed wellness resources, and physician-directed disease management services at no out-of-pocket cost to the employee.
