September 30, 2018 - Lynchburg, VA - One of Southern Virginia’s top clinically-integrated physician networks, Collaborative Health Partners of Virginia (CHP), today announced its success as a participant with one of the nation’s highest-performing accountable care organizations (ACOs), Privia Quality Network (PQN).
CHP’s primary care groups work to provide quality care at reduced costs for Medicare beneficiaries in Southern Virginia, which contributes to the successes of the entire ACO. As a group, PQN achieved shared savings of $38 million and a 95% quality score for its 2017 performance in the Medicare Shared Savings Program (MSSP). These achievements come from utilizing data analytics, care coordination, and care teams to deliver exceptional care.
“Success in shared savings programs comes from building a structure that helps patients get the right care, at the right place, and at the right time,” said Dr. Robert Elliott, Privia’s Regional Medical Director. “Having this structure in place helps ensure the providers and staff at CHP practices can focus more energies on patients.”
The infrastructure that helps CHP provide quality care to Medicare patients also improves care delivery for all patients. “Delivering high-quality, affordable care to our patients is a top priority for our team,” said Shawn Crawford, CEO of CHP and Central Virginia Family Physicians. “This recognition from CMS is a testament to the teams we have working to help all our patients. It’s incredible to see the dedicated and compassionate front office staff, clinical team, providers, and management all work together for the benefit of our patients and community.”
Among the practices in CHP are some of Lynchburg’s top primary care centers:
