The mission of Collaborative Health's Wellness Center is to establish an environment that promotes healthy lifestyles and enhances quality of life. We encourage people to strengthen their health and well-being through accessible health care, wellness coaching, and health education.

Acute Care

Our professionals are available to help you with the treatment of colds, flu, cough, fever, sore throats, abdominal pain, stomach upset, headache, bruises, cuts, sprains and strains, minor burns and other aches and pains.

Routine Screening

Patients have access to a variety of routine screenings, including cholesterol levels, blood glucose, blood pressure, pregnancy testing, body mass index, waist circumference, vision tests, EKG, fecal occult blood testing and depression screening. Review of results and recommendations for further exams or follow-up are provided as appropriate.

Wellness Coaching

Services include a voluntary health risk assessment and one-on-one counseling to implement behavioral changes to improve health. Educational seminars and literature are available.

Preventative Services

Prevention is the best medicine. Services include annual physicals, allergy injections, general immunizations, travelers’ health education, general self-care education, nutrition counseling and support and weight management. Referrals are provided to primary care physicians, specialists and the Employee Assistance Program as needed.
